Atlantic Drain Service Co., Boston, Massachusetts – $1,475,813 .

Bottom of excavation – Form

Two Atlantic employees killed – trench collapse.  Atlantic and company owner Kevin Otto charged with Manslaughter.

 

Arrow Plumbing, Blue Springs, Missouri – $714,142 .

Employee died in a trench collapse.  OSHA investigation showed employer still exposing its employees to trench dangers and cave-ins, issued six willful and eight serious violations.

Employees installing water-metering pits and lines; danger of trench cave-ins and other serious hazards.
